- Player Membership Form - Updated for Fall 06;complete player info only; parent & player to sign
- Medical Release Form - must be notarized, and complete player social security # required

Make sure to also submit your daughter's 1x1 head shot for their player card.  Passport photos are the perfect size.  "Kodak's" or "digital" pictures are NOT acceptable.
